Obtain an Accounting of Disclosures Regarding Your Personal Information
You may request a list of disclosures of your personal information, including those shared for legal or operational purposes. To request this information, contact our Privacy Office.
Authorize the Release of Your Personal Information or Appoint an Authorized Representative
You may grant authorization for someone else to access or manage your medical records on your behalf. To do so, please complete an authorization form available upon request.
File a Complaint If You Feel Your Privacy Rights Have Been Violated
If you believe your privacy rights have been violated, you have the right to file a complaint with Private Medical or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
Notify Private Medical About the Death of a Patient
If a patient of Private Medical has passed away, please notify us so that we can properly update our records and assist with any necessary arrangements.
Opt Out of Data Sharing Initiatives
Certain data-sharing initiatives may be used to improve healthcare services. You have the right to opt out of participation in such initiatives. Contact us if you wish to do so.

SMS/Text Messaging Privacy
Private Medical offers SMS text messaging so members can communicate directly with their care team.
Participation is voluntary and requires opt-in consent.
No mobile information, including phone numbers and SMS opt-in consent, will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors who support our service operations (such as messaging platform providers) is permitted solely to deliver the SMS service. All other use case categories exclude text messaging originator opt-in data and consent.
This information will not be shared with any third parties.
Members may opt out at any time by replying STOP to any message, or reply HELP for assistance.
